What is advance care planning?

Advance care planning (ACP) is an ongoing process of conversations that helps you think about, discuss and record what is important to you now and in the future. It enables you to make known your wishes, feelings, beliefs and values and make choices that reflect these.

 It can include:

  • your wishes, feelings, beliefs and values
  • your preferences for future health and care
  • who you would like involved in decisions about your care
  • legal and financial planning
  • practical plans for the future

Advance care planning can be started at any stage of adult life. Plans should be reviewed and updated as your circumstances change.

Who is advance care planning for?

Advance care planning is for all adults, regardless of age or current health. Although it is often associated with end-of-life care, it is much broader than this. Planning ahead can help you prepare for unexpected illness or changes in your circumstances, while giving those close to you a better understanding of your wishes and what matters most to you.

Why advance care planning is important?

Advance care planning can help you:

  • think about and communicate what matters most to you 
  • make informed choices about your future health, care and wellbeing 
  • help ensure future care reflects your wishes and preferences 
  • support family members and others who are important to you
