Children's Diabetes Service school log book: insulin injections

This log book is provided for use in schools with children and young people who have Type 1 diabetes and receive insulin injections. The log book is designed for recording blood glucose, carbohydrate consumed and insulin dosage throughout the school day. It has been distributed via the Children's Diabetes Service.
